Connection Forms

General Connection Form
This form applies to the majority of typical service connections. If the work being done is for a modification or repair please obtain a service modification form from the Commission. If this connection requires the use of manholes, multiple connection points, or is for an industrial use please obtain a Special or Industrial service application form from the Commission as needed. This form also includes the Septic System Status Form.
Special Connection Form This form is for new connections or for modifications to existing connections which require special permits. This form is generally required when the connection requires the use of manholes, has multiple connection points, or as otherwise required by the LSDC's. This form also includes the Septic System Status Form.
Service Modification Form
This form is required for any work being done to an existing service connection, such as the relocation or resizing of a service line.
Change In Use Form
This form is required whenever a facility already connected to the Commission's sewer is proposing any change that will result in a change of the characteristics or quantity of flow, or which changes the total number of originally bettered units.
Industrial Discharge Form
This is required for any industrial uses. In addition, industrial discharges will need authorization from the Massachusetts Water Resources Authority.
Flow Credit Transfer Form This form may be required in addition to the General and Special Connection Forms, Change In Use, and Service Modification forms. This will always be required for all new construction connections and in all other instances where the new connection is NOT replacing a failed septic system as determined by the Septic System Status Form or where a change in the existing service results in an increase of flow to the LSDC's system.

Information for Builders and Contractors

Drainlayer License Application
Required for all competent contractors wishing to make service connections to the LSDC's system.
Utility Contractor License Application
All sewer extensions, installations, repairs or maintenance work on sewer mains shall only be performed by a Contractor who possesses a valid Sewer Utilities License issued by the Commission.

These guidelines for site plans have been prepared to assist developers, builders, architects, engineers, and others in securing the necessary approval from the LSDC

Betterment Forms

Abatement Request
Use this form to submit a request to the Commission that your Betterment be waived in full. This form must be filed with the Board of Sewer Commissioners within six months from date of the notice of assessment. This is only used for newly bettered properties. If you believe that there is an error in your ongoing betterment repayment bill, please contact the LSDC's office.
Deferral Request A deferral is available to certain property owners where special circumstances, such as agricultural exemptions, warrant a payment process whereby the repayment of the betterment is postponed for a certain time period. This form must be filed with the Board of Sewer Commissioners within six months from date of the notice of assessment. This is only used for newly bettered properties.

A policy memorandum regarding the required timeframe for requesting an abatement.
Policy Memorandum - Betterment Prepayments
Policy regarding the calculation of interest after a prepayment has been made.
