These forms and other documents are provided to you by the Lancaster Sewer District as a courtesy. Although we make every effort to keep these documents updated they may change at any time without warning. All downloads are formatted as Adobe PDF documents unless otherwise specified. If you are downloading a form, please note that it must be filled out and mailed or delivered in person to the LSDC.   All associated fees and costs are in accordance with the LSDC’s Schedule of Fees.

Service Forms

LSDC Service Connection Multiform
This multiform is for all residential and non-residential new service connections and modifications. This is a universal form for all three classifications of service connections, General, Special, and Industrial. This form is required for any new service connection or for any work being done to modify an existing residential service connection, such as the relocation or resizing of a service line.  This form also incorporates a process for establishing the condition of a septic system and a process for issuing sewer capacity credits. 

Additional Sewer Connection Fees and Change in Use Fees as well as other fees may apply pending review of the completed application by the LSDC.

This document will assist you in calculating sewer flow capacity in Subsection C of the service connection multiform.

Usage Abatement Request Form
This form is used to request an abatement (reduction) in your sewer bill and to request an automatic sprinkler abatement.  There are numerous circumstances where an abatement will be considered.  The requested amount of reduction must be something that can be quantified.  It is the responsibility of the customer need to provide some compelling proof that the use is unusual and that you can reasonably calculate the unusual amount that you are seeking an abatement on.  Please attach additional documentation to this form when submitting.  All abatement requests are considered at first Commission meeting after submittal of the request.  It is recommended that you attend the meeting to discus your request.

Sewer Contractor Forms

Drainlayer License Application
Required for all competent contractors wishing to make service connections to the LSDC's system.

Utility Contractor License Application
All sewer extensions, installations, repairs or maintenance work on sewer mains shall only be performed by a Contractor who possesses a valid Sewer Utilities License issued by the Commission.

Betterment Forms

Abatement Request
Use this form to submit a request to the Commission that your Betterment be waived in full. This form must be filed with the Board of Sewer Commissioners within six months from date of the notice of assessment. This is only used for newly bettered properties. If you believe that there is an error in your ongoing betterment repayment bill, please contact the LSDC's office.

Deferral Request
A deferral is available to certain property owners where special circumstances, such as agricultural exemptions, warrant a payment process whereby the repayment of the betterment is postponed for a certain time period. This form must be filed with the Board of Sewer Commissioners within six months from date of the notice of assessment. This is only used for newly bettered properties.
